In order to comply with Texas House Bill 4224, the following information is provided:

Notice to Consumers –
Texas House Bill 4224 / Section 181.105
Effective September 1, 2025, Texas law requires health care providers, including licensed professional counselors, to prominently post detailed instructions for consumers regarding:

How to request access to your health care records;

How to contact the disciplinary or licensing authority for the provider; and

How to file a consumer complaint.

1. Requesting Your Health Care Records
You have the right to request access to your health care records maintained by this practice.
To request your records:

Submit a written request (signed and dated) to this practice. You can use my handy form here.

Your request should include:

Your full legal name,

Date of birth,
-
Contact information, and
-
A clear description of the records you are requesting (including approximate dates of service).

Requests may be submitted by email.

Your records will be provided in accordance with applicable Texas law and federal privacy regulations.

Reasonable copying, delivery, and administrative fees may apply as permitted by law.
If you are requesting records on behalf of someone else, documentation of your legal authority (such as power of attorney, court order, or other relevant authorization) must be included.
2. How to Contact the Licensing/Disciplinary Authority
The licensing and disciplinary authority for licensed professional counselors in Texas is the Texas Behavioral Health Executive Council (BHEC).
You may contact BHEC if you have questions about licensure standards, professional conduct, or the complaint process:

BHEC oversees professional licensing and discipline for LPCs and other behavioral health professionals in Texas.
3. How to File a Consumer Complaint
If you wish to file a consumer complaint regarding services provided by this practice, you can do so with the Office of the Texas Attorney General – Consumer Protection Division.
This office provides instructions and forms for submitting complaints about health care practices or other business practices in Texas.

Complaints should include as much detail as possible, including dates of service, names, and a description of the issue.
